Your care will the according to your need to help you regain those vital skills lost through disability or illness. We assure your quality of life will be improved through our re-enabling services as you regain independence and avoid prolonged hospital admission.
The period of this service varies. The service may last from a few days after being from hospital to your home or through the discharge process.
We will together with other professionals involve in your care to ensure a good transition from hospital to your home. We will provide you with qualified nurses, carers, and support workers to meet your agreed needs.
