Children’s Community Nursing Team
We provide support for children, young people, and their families with both acute and chronic medical and surgical conditions and complex health needs. We undertake a wide variety of nursing procedures and direct nursing care within the community setting.
We will act as the child's and family's advocate and can attend meetings with the family to offer support and liaise with other service providers.
We also offer an End-of-Life service which supports children, young people and their families in their wishes and choices in the child or young person's last stages of life.
The service is available for all children and young people registered with a GP in Nottingham City or Nottinghamshire County 0-18 years or 19 if in full time education.
The team consists of Children's nurses and a Learning Disability nurse who cover Nottinghamshire.
We also have a Children's community nursing and Learning disability team, based in Bassetlaw, consisting of Children's nurses, Learning Disability nurses and healthcare support staff. This service is available for all children and young people registered with a GP in Bassetlaw 0-18 years or 19 if in full time education.
We work alongside other agencies such as specialist nurses and therapy teams to ensure that children and young people, with any additional healthcare need, receive safe and effective care.

Referral
Babies who have, or are at risk of having developmental problems from early life are usually referred to the CDC by their neonatologist.
• Children who show developmental problems later in life are usually referred by their GP or health visitor to the locality community paediatrician who may then refer to the CDC if more specialist support is needed.
• Children with long term neurological conditions may be referred by the paediatric neurologists or other hospital based paediatricians.
• Referrals can come from any member of the wider child development team.
