Welcome to Meadowcroft
At Meadowcroft, we build our care and activity plans around an individual, supporting and encouraging people to be happy, cared for and fulfilled.
Making every day better is about achieving the best possible outcomes for those we care for, so we identify goals and outcomes together with individuals, families and professionals.
Our home
Service users at Meadowcroft live in one of two units, Connell and Acorn. Each unit has dining room, lounge and an adapted sensory bathroom. Bedrooms can be decorated to suit personal tastes and every room has an accessible ensuite wet room and TV and telephone points.
At Meadowcroft, we understand the importance of good, nutritious food. Our varied restaurant style dining menu is developed with service users to ensure a varied and interesting menu is available to everyone, taking into account personal preferences, condition-specific and religious dietary requirements.
A therapy kitchen with cooking and laundry facilities allows service users to develop the skills to live more independently.
Meadowcroft House also offers an extended pathway of care independent from the main home. This two-bedroom house is located on the same site and allows service users to live independently while still accessing care and support when needed. This service provides a rehabilitation pathway for those who have met their goals and are looking to return gradually to life in the community. It is also ideal for those with progressive conditions who can still live independently with support nearby.
Life at Meadowcroft
Social activities, leisure interests and hobbies are all part of a healthy, balanced life. Meadowcroft’s Skills for Life Co-ordinators and service users work together to create meaningful and individual plans. In addition, quarterly service user forums allow people to share their views and develop future activities. As well as group activities, staff facilitate individual hobbies and interests and where appropriate, support to access education and volunteering.
Day trips, dining out in the local community and shopping trips are popular, with service users encouraged to access local facilities to take part in their favourite sports and hobbies. We celebrate seasonal and cultural events and activities, and service users enjoy the cinema room and taking part in pool and snooker tournaments. Service users are supported to go on holiday and an accessible minibus means there is always transport available.
An enclosed patio garden has space for summer barbeques and events. Accessible for all, the garden includes areas for sitting and relaxing as well as gardening and socialising.
Friends, families and visitors
Our open door policy means friends and family are welcome to visit at any time and be involved in care planning and reviews. We encourage everyone to be involved in giving those we look after the best possible quality of life. A dedicated visitors room with a sofa and ensuite toilet is available.

Referral
Make a referral
Exemplar is a provider of choice and works closely with referring commissioners, local authorities and other organisations, offering a completely transparent process so referrers can track an individual's progress through each stage of their care.
To ensure that individuals are placed into a service most appropriate to their needs we have developed a robust and simple referral process.
Assessment
Treatment objectives and goals are agreed at the outset in agreement with the referrers’ requirements. This allows Exemplar to undertake an individual's pre-admission assessment.
During the assessment, information is gathered from many sources, including:
- The individual and their family
- Social workers
- Nurse assessors
- Previous placements
- Consultants and clinicians
- Therapists
This allows us to tailor our high-quality care plans so we can achieve the best possible outcomesfor individuals.
Working closely with referrers, we agree - wherever possible - goals and timescales, including the anticipated length of stay and plans for discharge. Every step of the way, we stay in touch with both the referring team and commissioner as an individual progresses through their care pathway.

- Call our 24 hour referral line on 01709 565700. Our referral team will provide you with all the information you need or put you in touch with the most relevant service.
- Alternatively, complete a referral enquiry form and we'll be in touch.
We aim to assess all referrals within two working days and provide costs within five days from initial enquiry. We can speed up this process for very urgent placements.
