ICS Aging Well - Clinical Frailty Scale App

Overview

As part of the Ageing Well plans for the Integrated Care System (ICS) one of the areas of focus is the identification of frailty. The Clinical Frailty Scale (CFS) provides a shared language between all areas, helping everyone to support older people to live well.
This new frailty training package is for: All people working in health and social care who support adults 65 and over across the Nottingham and Nottinghamshire ICS.
Why this training is required: To introduce the Clinical Frailty Scale (CFS) and ensure that the whole health and social care workforce across the Nottingham and Nottinghamshire ICS can communicate using a common language and so improve the quality of life for people living with frailty.

Clinical Frailty Scale and App now available
Clinical Frailty Scale (CFS) app is a free application that supports the Frailty Training Package outlined above. Frontline colleagues in all health and social care services can download for use on mobile devices, to undertake clinical frailty scoring with client groups in real time.
The CFS app works by responding with a simple Yes or No to a series of nine questions, starting at the most severely frail end of the scale. The response to the question identifies the level of frailty and provides a recommendation about how to approach that patient's care and treatment with extra information to read as needed.
