We offer a wide range of services, including nurse-led care, personal care, live-in support, reablement, respite, and transition services, all tailored to promote independence and enhance quality of life. Our office is located at the back of Pride Park, facing the railway track.
Referral
After a referral is submitted, we will schedule a meeting with a nurse after the 12-week mark to initiate a comprehensive care package tailored to provide the necessary support and assistance.
Please note that Empowering U packages require a minimum of four hours.
