Community Integrated Care - flexible and personalised approaches to Independent Living

Overview

Leicester & Stoke Supported Living

Community Integrated Care is one of Britain’s biggest and most successful social care charities. We support thousands of people who have learning disabilities, mental health concerns and autism to lead full, happy and independent lives in the community. We’re proud to provide Supported Living services across Nottinghamshire.

Our Supported Living services give people the opportunity to live independently in their own homes, with a bespoke package of support to get the most from life. They enable people to enjoy both a great living environment and the assistance of a great team.

We focus on really getting to know the people we support, so we can help them to achieve their personal goals. To do this, we work closely with the people we support, their families and the people who are important to them.

Our team can support you with:

  • Enjoying your hobbies and interests, and meeting friends
  • Developing domestic and life skills (such as shopping, cooking, cleaning)
  • Staying in touch with family and friends
  • Budgeting and managing finances
  • Getting out and about in the community
  • Attending college, work placements and social events
  • Accessing advice and advocacy
  • Keeping safe and healthy
  • Accompaniment to appointments

We also provide extra care services to support older people in their own homes, helping them to live as independently as possible in purpose-built accommodation. This is a home from home service, accessible 24 hours at the touch of a button. The level of support is dictated by those who use it and delivered in a bespoke way to suit the individual. Our staff are recruited from local areas to ensure that the people supported remain connected with their community.

We’re proud to have been recognised for our successful services at the likes of the National Care Awards, National Learning Disability Awards and 3rd Sector Care Awards.

To us, providing person-centred care is key. By recruiting colleagues who have great values, training them well and always responding to the direction of the people we support, we can provide exceptional support.
