The EHC Needs Assessment stage of the pathway involves collecting views, information and advice which will inform the decision as to whether or not an statutory EHC Plan is required.
Reports will be gathered from key people, including:
- Person with parental responsibility
- Child, Young Person
- Education settings
- Educational Psychologist
- Health professionals
- Social care professionals
- Other professionals who are involved
Once requested, the reports have to be returned to ICDS within 6 weeks.
If an EHC needs assessment is agreed, an Assessment Officer (AO) is allocated, The AO will offer to meet you early in the process to gather your views to feed into the assessment, a crucial part of the process involves enabling the child/young person and their family to tell their story they are also able to support your family with the completion of the ‘All About Me’.
The "All About Me" document sets out the child/young person's strengths and needs; their current levels of support; what is working well and what is not working so well; what is important to them; as well as their hopes and aspirations for the future.
You may wish to consider developing a multimedia wiki website, as part of the "All About Me Profile". The aim is to provide you and your family with the opportunity to use photographs, video, text, documents and web links to tell your story. Access to the wiki will only be available to those professionals identified by the family.
