A guide to applying online for a Short Break
Before applying for a Short Break please read the overview of the different levels of support available in the Service Statement. This document explains that the types of services and levels of support can vary in line with children and young people’s needs and circumstances.
If you need support to access the application
If you do not have online access or feel unable to complete the assessment yourself, then support can be provided by Ask Us Nottinghamshire.
Ask Us provide free, confidential and impartial information, advice and support around special educational needs and disability issues. There is a telephone helpline for callers to discuss the form with an advisor 0800 1217772 or you can email: enquiries@askusnotts.org.uk
PLEASE NOTE: When completing the online form the Applicant section requests the name, address and contact details of the person filling in the online application form. It is expected that this will be the young person themselves or a family member on behalf of a child/young person rather than a professional individual or organisation working with the family.
If your electronic device does not support the online application form
If you cannot utilise the online application form on your electronic device, please download the Short Break Application Form which can be found under downloads on this page, or request a hard copy from Ask Us Nottinghamshire at enquiries@askusnotts.org.uk or tel: 0800 1217772.
This form should then be completed and returned via email to sbassessment.review@nottscc.gov.uk or by post to Short Break Assessment & Review Team, Ground Floor, Meadow House, Littleworth, Mansfield, Notts, NG18 2TB.”
If your child is Electively Home Educated and under the age of 12
If your child is currently Electively Home Educated and under the age of 12, please download the Short Breaks Application Form which can be found under downloads on this page, or request a hard copy from Ask Us Nottinghamshire at enquiries@askusnotts.org.uk or tel: 0800 1217772.
This form should then be completed and returned via email to sbassessment.review@nottscc.gov.uk or by post to Short Break Assessment & Review Team, Ground Floor, Meadow House, Littleworth, Mansfield, Notts, NG18 2TB.
If your child is Electively Home Educated and aged between 12 and 17 years
If your child is currently Electively Home Educated and aged between 12 and 17 years please use the online application form.

Please note: The online Short Break application form asks for personal information about the child/young person. The form cannot be saved so the applicant will need to have the following information available in order to progress the application.
