Eligibility Criteria

A guide to how your application is scored

Each response to the questions on the application form generates a score.

It is these scores which determine whether the child/young person meets the threshold to receive a Nottinghamshire County Council funded Short Break. The criteria has been co-produced with families, and are shown below with the relevant scores:

  • Age of the child –if the child is aged 12yrs – 17yrs they receive 2 points. This is because parents and carers  have indicated that levels of need often increase as the child enters their teenage years, combined with fewer suppliers offering services to this age group.
  • Higher or middle bands Care Disability Living Allowance, or, higher disability living part of Personal Independence Payment (PIP), receives 3 points.
  • Higher level mobility Disability Living Allowance (DLA), or, higher mobility part of Personal Independence Payment (PIP), receives 1 point.
  • Education High Level Needs (HLN) receives 2 points
  • Education Health & Care (EHC) plan receives 2 points, but these are not additional to HLN points.

The threshold to receive a Nottinghamshire County Council funded Short Break is 5 points or more.

Below are some examples which illustrate how the points work in relation to the threshold for Nottinghamshire County Council funded Short Breaks; they are not representative of any existing or prospective Short Break applicants.

Example A

8 Year old child = 0 points

Higher level care Disability Living Allowance (DLA) = 3 Points

Total Points = 3 Points - Threshold not met for funded Short Break.

Example B

6 Year old child = 0 points

Higher level care Disability Living Allowance (DLA) = 3 Points

Education Higher Level Needs (HLN) = 2 points

Total Points = 5 Points - Threshold met for funded Short Break.

Example C

12 Year old child = 2 points

Education, Health and Care (EHC) Plan = 2 Points

Total Points = 4 Points - Threshold not met for funded Short Break.

 

Example D

16 Year old child = 2 points

Education, Health and Care (EHC) Plan = 2 Points

High level Personal Independence Payment (PIP) = 3 Points

Total Points = 7 Points - Threshold met for funded Short Break.
