A guide to how your application is scored
Each response to the questions on the application form generates a score.
It is these scores which determine whether the child/young person meets the threshold to receive a Nottinghamshire County Council funded Short Break. The criteria has been co-produced with families, and are shown below with the relevant scores:
- Age of the child –if the child is aged 12yrs – 17yrs they receive 2 points. This is because parents and carers have indicated that levels of need often increase as the child enters their teenage years, combined with fewer suppliers offering services to this age group.
- Higher or middle bands Care Disability Living Allowance, or, higher disability living part of Personal Independence Payment (PIP), receives 3 points.
- Higher level mobility Disability Living Allowance (DLA), or, higher mobility part of Personal Independence Payment (PIP), receives 1 point.
- Education High Level Needs (HLN) receives 2 points
- Education Health & Care (EHC) plan receives 2 points, but these are not additional to HLN points.
The threshold to receive a Nottinghamshire County Council funded Short Break is 5 points or more.
