Community Learning Disability Teams FAQs

What is a Community Learning Disability Team (CLDT)?

They are a group of Specialist Learning Disability Nurses based in the Community Learning Disability Teams (CLDT's).

Their service is for adults (18-65) with a diagnosis of an intellectual disability. They work closely with other health professionals.

Community nurses can help with:

  • emotional and behaviour needs
  • epilepsy
  • health screening
  • understanding your own health needs
  • help with medication
  • healthy eating
  • getting the right help you need from all health services
  • health action plans

They will talk to you about the help you need and how we can support you. This may include:

  • working with your family and support staff
  • giving you advice
  • working with Adult Services and other agencies
  • Working with other health professionals e.g. doctor, hospital, dentist etc

What age can my young person start working with the Community Learning Disability Team?

CLDT's will start working with adults from the age of 18. 

what is the equivalent of the Community Learning Disability Team for children?

Children's Nursing Services

The Children's Nursing Services provide a range of nursing services for children and young people who have a learning disability, physical disability and/or a complex health condition.

These services include:

  • children's community nursing
  • short breaks, day care and overnight short breaks for children living in Nottingham City
  • a range of paediatrician led clinics

In Mansfield and Ashfield and Newark and Sherwood, they offer a community nursing service to children with complex health problems, disability and also learning disability.

The services are available to children and young people aged 0 – 19.

Community Children's Team - Bassetlaw

The Children’s Learning Disability Team accepts referrals from any source and provides specialist advice and strategies that can be implemented by carers at home, school and other community settings for children with moderate to severe learning disabilities. Their expertise supports the child, their families, carers and schools in areas such as sleep, continence and behaviour management, in order to create the appropriate resources and environment for the child to achieve their best potential.
