What is the Active Exercise Referral Scheme?
The Active Ashfield Exercise Referral Scheme is delivered in partnership by Ashfield District Council and Everyone Active. The scheme aims to encourage referred patients to become more active to improve their overall health and wellbeing.
All activities are delivered by trained Exercise Referral Instructors who will create an exercise programme based on your individual needs and interests. A variety of exercise options are available including gym sessions, body conditioning, aqua and much more. All participants must be referred onto the scheme by a medical professional e.g. GP, Practice Nurse, Physiotherapist using one of our referral forms.
Am I eligible for the scheme?
Patients can be referred onto the scheme for any of the following:
- Asthma
- COPD
- Muscle – Skeletal
- Inactivity (less than 30 minutes activity per week)
- Diabetes
- Stroke (specific sessions are available for this group)
- Arthritis/Rheumatism/Osteoarthritis
- Smokers
- High Blood Pressure
- Overweight/ Obese
- Stress/ Depression
- Coronary Heart Disease
All participants must be aged 16 years and over
How much does it cost?
All sessions are available at a discounted rate of £2 per session for a period of 12 weeks. After the 10 week period participants are encouraged to continue exercising with discounted ‘pay as you go’ and membership options available.
