The Family Nurse Partnership Programme
The Family Nurse Partnership Programme (FNP) is an intensive home visiting programme for first time teenage Mum’s aged 19 and under. Trained Family nurses work with clients from early pregnancy up until their baby is 2 years of age, with an aim of improving parenting, building self-efficacy, improving child development and school readiness, and enabling Mum’s to become economically self sufficient.
FNP is an evidence based programme and is provided by Nottinghamshire Healthcare NHS Trust

- Referral Details
Referrals can be made by any organisation. In order to access the programme, clients must be eligible according to the following criteria.:
- 19 or under at last menstrual period.
- Expecting their first child
- Living in Nottinghamshire
- Registered with a Nottinghamshire GP
- Be less that 24 weeks pregnant at the time of referral
