If you have a learning disability and live in Nottinghamshire there is a variety of support available from individuals and local organisations. 

How to get support?

If you think you need social care support, start by looking at our Nottinghamshire Help Yourself website. Here you will find information and advice about support for adults with a learning disability, as well as search for local organisations that can provide care services. 

The Learning Disability Partnership Board being people together to make services better. 

Our Community Learning Disability Teams provide specialist assessment, advice, treatment and support services for adults with a learning disability and their carers. We have an easy read factsheet about their services.

Our Promoting Independence Workers can help you to become more independent by supporting you to develop daily living skills, use public transport or take part in activities to meet your needs. The aim is to improve your confidence and skills so you can eventually achieve these things without support.

How to find work

Our iwork team can support people with a learning disability to find employment.

Residential care

If your needs are greater, and if you are eligible, you may need to consider residential care. Our Care and Support Directory has a full list of care homes for younger adults.

We also run our own residential care home for adults with learning disabilities called Church Street.

The teams have the remit of being the gate-keeper to services, providing a seamless, needs-led service to adults with a learning disability living in the community. There are 10 CLDTs that cover the whole of the county with the following bases:

Referral Details

In the first instance, new contact for Adult Social Care should be made with the Customer Service Centre 0300 500 80 80 who will try to resolve any concerns the person may have.If they are unable to help, the person may be referred onto a more appropriate service.

To access the service, the person has to have a learning disability, be over 18 years of age or in transition from children’s services or education.

Assessments, support plans and agreements for funding will be made where necessary. Each team has a duty worker who can make contact with urgent cases.

Some people may already have involvement and support from a local CLDT. If this is the case, the person may be able to talk to a professional they already know, or a duty worker, to help access other services and support.

Referrals for support from the health professionals can be made via a person’s GP, from support providers or community services. They will also need to contact the Customer Service Centre.

In some cases the person may be referred onto another team or professional where the support may be better suited or specialised.

Description

The health professionals within the team will provide information and advice to adults with learning disabilities covering a wide spectrum of health related issues.

They are often best placed to provide support and assistance in the end stage of an individual's life due to the existing relationships and knowledge within the framework of the CLDT. At this often difficult, but vital stage of an individual's life, the health focus of the team will be on supporting the individual and their carers to understand the processes involved and providing therapeutic interventions to assist with both the physical and emotional implications.

They will be integral to the liaison amongst wider members of the multi-disciplinary team and to the facilitation of all required planning and implementation of the individual's care needs.

The community learning disability team is available as a resource to the individual and their carers, to enable informed, considered and fairer access to health care at a time when it is needed most.
