Adult Social Care & SEN: The Sustainable Land Trust

We offer a unique range of services using the natural landscape around us to improve health, wellbeing, skills, and education.  We are experienced in working with SEND adults and young people to help them achieve their goals and enable them to gain qualifications. We utilise the beautiful landscape around us and our field study centre to help reduce mental and physical health issues and improve social skills. Being in nature helps people to make friends, overcome challenges, work positively, and improve their confidence to try new things.

People benefit from learning and experiencing nature through wildlife and environmental monitoring, practical habitat management, creating natural crafts, gardening, estate management and construction, and running small enterprises (foods, fuel and crafts). We have supported over 65 adults and young people since 2015; some go into employment or further education; some stay with us to benefit from the services that nature provides.

A peaceful rural setting close to Melton Mowbray and on a bus route.  We are unable to provide transport but accept clients from a very wide area.  We are working towards making our premises more disabled friendly but cannot currently offer disabled access.

Referral Details

We accept referrals from Adult Social Care, Continuing Health Care and from clients funding their own care. Referrals cannot be provided without an agreed package of Local Authority or DWP support.  All clients should be between 14 and adulthood and have a Statement of Special Educational Needs (SSEN), Educational Health Care Plan (EHCP), Personal Independence Payment, Disability Living Allowance or Self-Directed Support (personal budgets, individual budget, direct payment, or personal health budget) - please forward details with referral email.

 

We support individuals to achieve their goals, believing passionately that every person, regardless of the challenges they face, can accomplish extraordinary achievements with the right support. Giving a person with the support is of paramount importance to us. We offer a responsive initial assessment to identify the individual’s needs and how we can support them.

 

We work with clients and their advocates to plan a transition programme that will allow people to get the most out of their time with us.  In each case, we assess the needs, abilities and personality of the client in detail, to determine their suitability for our various areas of activity or education course.
