ICDS - Specialist Support Team (SST)
The Specialist Support Team offers bespoke, individualised packages of support for children and young people aged 0-18years, either within their own homes or local communities, delivered by trained / experienced support workers. The team is registered and inspected by Care Quality Commission (CQC)
The team offers:
- Home-based Support
- Assessment & Delivery of Personal & Intimate Care (PIC) in the home
- 1:1 Sleep and Routines interventions in the home - after a graduated response
- Joint working with OT to promote confidence in caring
- Building independence and self-care skills
- Community Support
- Short breaks in a group setting
- Individual short breaks to transition into a group for complex needs
- Targeted short breaks in our bases, for those who are unable to manage a group
- Off site group sessions in the community to extend young peoples experiences
- Targeted Support - Via a Childrens Disability Service (social care team) referral.
- Direct work with young people - individually tailored to that young persons needs
- Family support, working in a family focused way
- Targeted youth group for young people working towards a more universal provision.

- Description
The Specialist Support Team offers:
- Personal Care needs Assessment:
- We provide an assessment of the personal care needs of children / young people, where they have personal care tasks which require support from 2 or more adults and there is insufficient adult capacity within the family to undertake these tasks. There tasks are usually moving and handling related
- Establishing Positive Routines:
- We offer children / young people and their families, practical and emotional support with establishing routines and practical ways to manage behaviour that may be related to these routines, such as morning or evening routines.
- Sleep support and advice
- We provide accredited Sleep Practitioners who support children / young people and their families to establish a healthy sleep routine. This could be alongside overnight support in the home if required to improve the outcomes of the support.
- Short Break Provision
- We provide specialist sessions for young people with complex needs, who are unable to access other universal provision. These run in an appropriate environment which enables the young people to have fun, have social interactions and engage with new group experiences. These sessions are supported by staff with specific knowledge, experience and training, matched to the needs of the group.
*The Specialist Support Team may signpost families requiring support for younger children to more age appropriate services such as Childrens Centres
