By this point the Education, Health and Care (EHC) Plan will have been finalised. The content of the plan will have been shared with all those who were involved in its development, to ensure that everyone is clear what is expected of them. This is also about ensuring that appropriate plans are in place to enable the child/young person to achieve the outcomes they want for themselves.

If your family have developed a Wiki, then this can be used to demonstrate success and  achievements. If the young person wishes then the Wiki itself can continue beyond the age of 25 (the upper age or an EHC Plan) and serve as a support tool for life.

If you have opted to take elements of a personal budget as direct payments, then you will be given support and advice to help achieve the best results from the resources available.

You and your family will continue to be involved in reviewing and contributing to the ongoing development and implementation of the EHC Plan.
