Preparing for Adulthood

This section of the Local Offer is aimed specifically at young people from 16 - 25. It is intended to provide information about the transition from children's services to adults. Nottinghamshire has a new multi-agency transitions protocol and pathway, which is the process and journey that young people with disabilities or additional needs transfer from Children's Services to Adult’s Services in Education, Health and Social Care provision. These documents can be viewed under the downloads section.

Please see the related links and advice sections for additional information about specific areas that may be of interest.

We have included a link to a video produced by the Council for Disabled Children which explains what you need to know about the SEND reforms and post-16 support.

For information about training, apprenticeships, supported employment and advice about independent living, please see the advice and guidance section of this page or visit the Employment and Training and Independent Living sections of the site using the icons on the Local Offer home page.
