Preparing for Adulthood

This section of the Local Offer is aimed specifically at young people from 16 - 25. It is intended to provide information about the transition from children's services to adults. Nottinghamshire has a new multi-agency transitions protocol and pathway, which is the process and journey that young people with disabilities or additional needs transfer from Children's Services to Adult’s Services in Education, Health and Social Care provision.
